    Please provide any financial documents that can support your Statement of Need, (ex. Profit & Loss Statement, Tax Return, Bank Account balance sheet).
     

    Proof of dissolution, if applicable. (This is a document obtained from the Secretary of State confirming that you have dissolved your business).
